The potential program participant is evaluated for suitability, and the staff member responsible for admissions into the house conducts the initial screening.
The staff will collect assessment information and confirm the program participant's identity using the participant's state-issued identification and social security card. Should the program participant lack identification, the staff will provide assistance to initiate the process of obtaining identification.
The staff shall review the house confidentiality policy, request the program participant to sign the confidentiality agreement, and complete the appropriate release of information forms.
New program participants will be informed of the following:
- Recurrence or Relapse policy
- Program Participant rights and responsibilities
- Fee agreement
- Description of services offered
- Curfew
- House Rules
- Recovery Goals
- All documents will be signed and dated by the program participant and staff. The program participant will be given a copy of the signed documents for their further review and reference.
- If the program participant has not had a recent drug screen/breathalyzer, the House Manager will arrange for a urine drug screen, breathalyzer, and/or cheek swab. (This may be done on site or at an offsite testing facility depending on the expertise and training of the recovery staff.)
- The participants' personal stuff will be checked over. Anything that doesn't help recovery or a safe, cozy, drug- and alcohol-free space will be thrown out.
- You'll need to have a current physical and lab work done, including a TB test. The Beyond The Horizon staff will check to make sure you've scheduled an appointment with your doctor or the local health department. They'll also guide you on getting the documentation for the physical or lab work, or help you fill out the right Release of Information form so they can get the test results from the referral source.
- The participant will have a detailed overview of the house rules which must be signed by you. You will be given a room, introduced to the housemates, and shown around the residence. If you have any questions then, feel free to ask and we will happily answer and make you feel at home.
